Purpose of This Notice

This notice describes how health-related information may be handled if encountered during advisory services provided by Bridge the Gap™ and explains important limitations regarding the submission of patient information through this website.

Bridge the Gap™ is a clinical advisory program. It does not provide patient care, diagnosis, treatment, billing, or insurance services.

Covered Entity Status

Prestige Dental Consultants, LLC and Bridge the Gap™ are not HIPAA-covered entities and do not act as health care providers, health plans, or health care clearinghouses as defined under the Health Insurance Portability and Accountability Act (HIPAA).

Accordingly:

  • We do not create or maintain designated medical records.

  • We do not request or require patient-identifiable health information (PHI).

  • We do not provide Notice of Privacy Practices as required of dental or medical offices.

Handling of Health Information

In the course of advisory engagements, it is possible that Bridge the Gap™ may incidentally encounter limited health-related information while observing systems, workflows, or documentation within a dental practice.

Any such information:

  • is treated as confidential

  • is not retained beyond the scope of the engagement

  • is not used for patient care, diagnosis, or treatment

  • is not disclosed except as required by law

Bridge the Gap™ evaluates systems, not individual patients.

Website and Form Submissions

Please do not submit patient-identifiable health information through this website, including but not limited to:

  • names

  • dates of birth

  • medical or dental histories

  • diagnostic images

  • insurance details

This website is not designed to receive or store protected health information.

Confidentiality of Advisory Work

Information shared by a dental practice during an engagement is handled confidentially and used solely for advisory purposes. This includes operational, staffing, and systems-level observations.

Confidentiality obligations are further governed by the applicable consulting agreement between Prestige Dental Consultants, LLC and the practice.

Electronic Communications

Email and web-based communications are not guaranteed to be secure. By contacting Bridge the Gap™ electronically, you acknowledge and accept this limitation.

If sensitive information must be shared, alternative methods should be discussed directly.

Legal Disclosures

Bridge the Gap™ may disclose information if required to do so by law, court order, or regulatory authority.
